Mabel Slusher Forman

Mabel Slusher Forman, age 90, of Clearfield, passed away Friday, September 18, 2026, at her home. She was born March 18, 1936, at Craney in Morgan County to the late Sam Slusher and Lizzie Pettit Slusher. In addition to her parents, she was preceded in death by her beloved husband of 60 years, Walter Forman, on May 19, 2016.

She leaves five children to honor and cherish her memory: Anita Broomall, Georgetown; Diana (Mitch) Hatton, Clearfield; Jean (John) Mahaney, Clearfield; Lisa (Brent) Back, Clearfield; Eric (Sarah) Forman, Nicholasville; twelve loved grandchildren: Michael (Lauren) Broomall, Lincoln University, PA; Risa Shafer (Danny) Boyd, Morehead; Chris (Megan Wallace) Shafer, Clearfield; Jade Shafer Jones, Pompano Beach, FL; Collin Alexander, Clearfield; Canon Mahaney, Portsmouth, OH; Braden (Ashley) Back, LaGrange; Cade Back and Piper Back, both of Clearfield; Cory Forman, Nicholasville; Emily (Spencer) Wadlington, Lexington; and Nora Forman, Nicholasville; six special great grandchildren: Hayley Boyd and Camron Boyd, Morehead; Audrey Jones and Gretta Jones, Pompano Beach, FL; Brennix Alexander, Clearfield; and Evelyn Back, LaGrange.

She also leaves behind special friend Lynda Reynolds and all her church and volunteer family; the parents of her grandchildren: Jennifer Forman, Nicholasville; Will Alexander, Salt Lick; Bruce Broomall, FL; her dog, Pepper, and her many pet chickens.

Mabel was a homemaker and sold Avon for many years, but she was also very active in the community, with her volunteer work starting back in the 1970s when she walked around the neighborhood on Clearfield Hill going door to door requesting donations for the American Heart Association and the American Cancer Society. When her children were in school, she served as a chaperone for many class field trips and band competitions. Mabel and Walter both tutored at Clearfield Elementary School in the 90’s serving as reading coaches. She was a volunteer for the 4-H council, served as 4-H Camp Adult Counselor, and Girl Scout Leader. She was always up to help with any community service projects that her Homemaker's Club was involved in. Mabel began working with the RSVP volunteers at the school board office in August of 2019, but she began volunteering for Rowan County schools and our community long before that. She has dedicated 22 years of service as a volunteer and earlier this year was honored with the AmeriCorps Lifetime Achievement award, which she was very proud of. Mabel was a faithful member of the Clearfield Tabernacle First Church of God where she was a Sunday School teacher and a childcare/nursery provider during services. Her smile and contagious laugh will be missed by all that knew and loved her.

Funeral services will be conducted at 11 a.m. Thursday, September 24, 2026, at Northcutt & Son Home for Funerals, with services being provided by Pastor Roy Lands, Pastor Saundra Nathanson, and Collin Alexander. Burial will follow in Crix Cemetery.

Pallbearers will be Collin Alexander, Braden Back, Brent Back, Cade Back, Cory Forman, and Canon Mahaney,

Honorary pallbearers will be Michael Broomall, John Burton, Mitch Hatton, John Mahaney, Chris Shafer, and Ricky Story.

Visitation will be from 6 p.m. to 8 p.m. Wednesday, September 23, 2026, at Northcutt & Son Home for Funerals, 400 Fraley Drive, Morehead, Kentucky.
