Douglas Gladof Forman, age 88, of Kentucky Highway 519, Morehead, passed away Friday, May 6, 2016, at St. Claire Regional Medical Center in Morehead. Born March 31, 1928, in Clearfield, he was a son of the late Goldman G. and Gladys Hall Forman. Besides his parents, he was preceded in death by one son, Barry Douglas Forman, and two brothers, Larry Forman and Kenneth Forman. Doug is survived by his wife, Ethel Dolly Fugate Forman; one son, Douglas Garland 'Dougie' Forman of West Liberty; one sister, Dorothy Armstrong of Morehead; four grandchildren, Brandy Forman, Shawn Forman and wife Kassandra, Leslie Lewis and husband Tom, David Forman and wife Teresa, all of Morehead; eleven great grandchildren; several nieces and nephews; and many friends. Doug was a retired parts salesman, working over 60 years at Wholesale Auto Parts and Rowan Motor Sales. He was a member of Morehead Masonic Lodge No. 654, Morehead Shrine Club, and El Hasa Shriners in Ashland. He enjoyed playing the guitar, listening to rhythm and blues music, and target shooting. Funerals services will be conducted 2 p.m. Tuesday, May 10, 2016, at Northcutt & Son Home for Funerals Memorial Chapel with the Rev. Jeff Fannin officiating. Burial will follow in Forest Lawn Memorial Gardens. Pallbearers: Denver Forman, Randy Forman, Matt Forman, Daniel Rogers, Phillip Barber, and Raymond Perry. Visitation is 5 p.m. Monday, May 9, 2016, with Masonic services to be conducted at 7:30 p.m. Monday by Morehead Masonic Lodge No. 654 at Northcutt & Son Home for Funerals in Morehead.
